Working Permit

• Instructions for students under 16 years old:

  • Print and fill out the following forms:

    • Special Limited Permit to Work form. The student must sign this form.

    • Intention to Employ A Minor form. The employer and guardian must sign this form. 

• Instructions for students 16+ years old:

  • Print and fill out the following forms:

    • Certificate of Age form. The student must sign this form

    • Intention to Employ A Minor form. The employer and guardian must sign this form. 

• Please submit all signed paperwork to the Main Office.

• Once the forms are prepared, we will contact you to pick up copies for you and your employer.
